Eczema (also known as atopic dermatitis) is a chronic skin condition that can affect individuals of all ages. It commonly presents as red, itchy, inflamed, or scaly skin. In some cases, it can disrupt sleep or daily activities (particularly in children) and may require medical management.

Eczema is associated with a genetic predisposition and involves a combination of skin barrier dysfunction and an overactive immune response. This cycle can make the skin more sensitive to environmental irritants and allergens, leading to worsening symptoms over time.

There are several recognised types of eczema or dermatitis, including:

  • Irritant Contact Dermatitis – caused by repeated exposure to soaps, solvents, or friction
  • Allergic Contact Dermatitis – a delayed immune response to specific allergens
  • Asteatotic Eczema – often seen in older adults, linked to dry or cracked skin

Eczema presents as a rough, red, scaly or bumpy rash. When severe it can become weeping and crusted. The most common areas affected are the flexures (inside elbows and behind knees) and the head and neck in children, but eczema commonly affects all other body areas. It is extremely itchy for most people.

Eczema triggers vary among individuals, but common exacerbating factors include:

  • Stress: Emotional stress can worsen symptoms.cityskinclinic.com
  • Weather: Cold, dry climates or sudden temperature changes can trigger flare-ups.
  • Environmental Allergens: Dust mites, pollens, and pet dander are common culprits.
  • Skin Irritants: Soaps, detergents, and certain fabrics can irritate the skin.
  • Infections: Bacterial overgrowth on the skin can exacerbate eczema.

Treatment for eczema varies depending on the type, severity, and location of the condition, as well as patient age and personal preferences.

A key part of managing eczema involves identifying and avoiding factors that may trigger or worsen symptoms, while supporting the skin barrier to reduce the risk of flare-ups. Inflammation also plays an important role in ongoing skin damage and may require active treatment to help the skin recover.

Management approaches may include:

1. Prevention and Protection of the Skin Barrier

  • Avoiding irritants such as soaps and solvents that may disrupt the skin’s natural protective barrier
  • Minimising exposure to physical irritants like sand or prolonged water contact
  • Managing temperature extremes and avoiding overheating, especially in children
  • Applying bland, fragrance-free moisturisers and barrier creams regularly to maintain hydration
  • Using wet wraps or dressings to reduce scratching and relieve itch in affected areas
  • In some cases, dilute bleach baths may be used under medical supervision to reduce bacterial overgrowth
  • Skin care should be selected to suit eczema-prone skin and support the skin barrier

2. Reduction of Inflammation

  • Topical corticosteroids, as prescribed by a medical professional, to help manage inflammation and reduce itch
  • Wet wrap therapy as a supportive method to assist with absorption of topical treatments
  • Narrow-band UVB therapy may be considered in certain cases under specialist guidance
  • For more severe eczema, systemic therapies such as oral corticosteroids or steroid-sparing medications may be discussed
